Terms & Conditions

Terms & Conditions Capability Practice Ltd Last updated: 15 October 2026 These Terms & Conditions explain how our private Occupational Therapy services are arranged and delivered, including bookings, fees, cancellations, clinical care and your rights. Please read them before booking a paid service. 1. About Capability Practice & Our Services About us Capability Practice is operated by Capability Practice Ltd, a company registered in England and Wales. Company number: 15278776 Registered office: 2nd Floor College House, 17 King Edwards Road, Ruislip, London, United Kingdom, HA4 7AE We provide private, home-based Occupational Therapy services in Banbury and surrounding areas. Our services Complimentary Initial Consultation — Free An opportunity to discuss your circumstances, ask questions and explore whether our services may be suitable. This consultation is not a comprehensive clinical assessment. Initial Occupational Therapy Assessment — £325 A personalised home assessment lasting up to 90 minutes, followed by a written summary of findings and recommendations. Follow-Up Occupational Therapy Intervention — £120 A 60-minute home visit providing personalised intervention, practical support or review based on your assessed needs. Functional Longevity — £3,000 A structured 12-week programme including an initial Occupational Therapy assessment and 24 therapist-led home visits of 60 minutes each, normally delivered twice weekly. The programme also includes personalised intervention planning, ongoing clinical review and a final reassessment as part of the intervention. Clinical suitability Our services are recommended according to individual clinical needs, circumstances and goals. We may recommend an alternative service, further assessment or referral to another healthcare professional where this would better meet your needs. We may decline to begin or continue an intervention where it would be clinically inappropriate or unsafe. We will explain our reasoning and discuss appropriate next steps. We do not guarantee specific clinical outcomes.

Booking appointments An enquiry or complimentary consultation does not automatically constitute a confirmed booking. Paid appointments are confirmed once we have agreed the service, appointment details, applicable fees and any relevant booking requirements. Before enrolment in Functional Longevity, we will confirm the programme schedule, total investment, payment arrangements and any additional travel charges. You will receive the relevant booking information and these Terms & Conditions before entering into a paid agreement. Fees and payment arrangements Our standard fees are: Initial Occupational Therapy Assessment: £325 Follow-Up Occupational Therapy Intervention: £120 per 60-minute appointment Functional Longevity: £3,000 for the agreed 12-week programme Payment instructions and due dates will be provided when your booking is confirmed. Functional Longevity may be paid in full or through an agreed instalment arrangement. The payment schedule will be confirmed in writing before the programme begins. Instalments contribute towards the total agreed programme fee; they are not separate purchases of individual appointments. Any additional charges will be explained and agreed in advance. Functional Longevity assessment credit If you have already completed an Initial Occupational Therapy Assessment with Capability Practice and subsequently enrol in Functional Longevity, the £325 assessment fee may be credited towards the £3,000 programme investment. Where the full credit applies, the remaining programme balance is £2,675. The assessment must remain clinically relevant to planning your programme. If your circumstances have materially changed, further assessment may be required before intervention begins. The assessment credit can only be applied once and does not reduce the total programme investment below £3,000. Home visits and travel Travel within 10 miles of Banbury is included in our published fees. Appointments beyond this area may be available, generally within approximately one hour of Banbury, subject to availability and suitability. Additional travel charges may apply outside our core service area. These will be explained and agreed before booking.

Cancelling or rearranging individual appointments We ask for at least 48 hours' notice if you need to cancel or rearrange an appointment. We recognise that illness, hospital admission and unexpected circumstances can affect appointments. We will consider these situations fairly and discuss appropriate arrangements. Any late-cancellation or missed-appointment charge will be communicated before booking and will be fair, proportionate and consistent with applicable consumer law. If Capability Practice needs to cancel an appointment, we will offer an alternative appointment or an appropriate refund for services not provided. This policy does not affect your statutory cancellation rights. Changes to Functional Longevity Functional Longevity is designed as a structured 12-week intervention, with appointments scheduled in advance. If illness, hospital admission or a significant change in circumstances interrupts your programme, we will review whether it remains appropriate to continue, pause or modify the intervention. Any revised schedule or arrangements will be agreed in writing. Ending a programme early You may request to end Functional Longevity before completing the programme. We will explain the financial implications, taking account of services already delivered, reasonable costs, the circumstances of termination and your statutory rights. We do not operate a blanket no-refunds policy. Where Capability Practice ends a programme because continued intervention is clinically inappropriate or because we cannot deliver the agreed service, we will provide an appropriate refund for undelivered services, subject to applicable law. Your statutory cancellation rights Depending on how your service is booked, you may have a legal right to cancel within 14 days of entering into the contract. Where this right applies, we will provide the required cancellation information before you are bound by the agreement. If you expressly request that services begin during the statutory cancellation period, you may be required to pay a proportionate amount for services already supplied if you subsequently cancel, in accordance with applicable law. Any request to begin services early, and any acknowledgement required by law, will be obtained separately. You can notify us that you wish to cancel by emailing contact@capabilitypractice.co.uk or by making another clear statement. Nothing in these Terms & Conditions removes or restricts your statutory consumer rights.

Consent and participation We will explain the nature and purpose of proposed assessments and interventions and seek appropriate consent. You may ask questions, decline a particular intervention or withdraw consent. We ask that clients provide relevant information about their health, medications, changes in function and any concerns that may affect safe intervention. Where appropriate, family members, carers or representatives may be involved in discussions and planning, with the necessary consent or other lawful authority. Home visit safety We ask that the home environment is reasonably safe and accessible for the planned appointment. Where a significant safety concern arises, we may need to modify, postpone or stop an intervention. Capability Practice does not provide emergency medical services. If urgent medical assistance is required, please contact the appropriate emergency service. Although we work towards meaningful improvements in everyday capability and independence, specific outcomes cannot be guaranteed.

Your statutory consumer rights remain unaffected. Changes to these Terms & Conditions We may update these Terms & Conditions from time to time.
